Measure 3326

How many participants who were directly provided with services obtained housing?

RequiredoutputQ80

Tier 1: Collect today

A count of services or people, held in the organization’s own records.

Housing can include transitional housing, such as group homes and long-term shelters. Include any participant who is not homeless or without a place to stay. Report on individuals, not the number of times housing was obtained. A. For participants receiving post-release services: The next set of questions is about court and criminal involvement for ex-offenders who have ever participated in the Second Chance Act (SCA) co-occurring program. Note that individuals can be included in more than one category. For example, an ex-offender could be arrested and have a conviction; thus, the individual should be counted in both categories. If an ex-offender has multiple arrests or convictions, only count that individual once. This section is to be completed at the close of the grant.
