RequiredotherFY2026Q7

Tier 2: Add one free instrument

Asks for a reported change in knowledge, attitude, behavior or condition.

Instruments for substance use
  • Brief Addiction Monitor, BAM (Continuous version) Public domain 17 numbered; substance subitems within item 7 items, about 5-10 min

License: “The BAM is in the public domain.”

EN verified. Use the continuous-version key; VA retired its categorical version. No overall "recovery total."

    Form
  • AUDIT Free, with conditions 10 items, about 3-5 min

License: “No permission is needed for any non-commercial purpose.”

EN; more than 40 translations listed. Preserve content, do not charge respondents, and identify it as WHO-approved. Standard past-year recall makes closely spaced pre/post comparisons difficult.

    Form
  • Substance Use Recovery Evaluator (SURE) Free, with conditions 21 scored, plus unscored priorities section items, about 5-10 min

License: “use within not-for-profit charities or health care settings does not require a licence.”

EN verified. Distribution, commercial exploitation and electronic storage/transmission have separate restrictions. Particularly relevant to nonprofit paper administration; do not assume permission to embed it in your software.

    Form
  • AUDIT-C Screening only 3 items, about 1 min

License: “available for use in the public domain.”

EN verified. Versions change the heavy-drinking threshold; retain the exact version and scoring rule.

Unique means unduplicated. Individuals should only be counted once. Unique individuals should not be counted in more than one reporting period. Individuals refers to youth program participants. Program participant refers to an individual who started a program. Discharged means the program participant is no longer participating in services or receiving services directly from or indirectly through your program. Unsuccessful discharges refer to individuals discharged for a reason other than successful, such as a discharge due to loss of contact, removal, or new petition. • Loss of contact means a youth in the program stops communicating with the program or fails to attend required appointments. • Removed means the exiting program participant is no longer participating in the program for failing to follow program rules or treatment recommendations, such as consistently missing appointments, abusive behaviors, or lack of engagement. • Petition is a document filed in juvenile court alleging that a juvenile is a delinquent or a status offender and asking that the court assume jurisdiction over the juvenile or that an alleged delinquent be transferred to criminal court for prosecution as an adult. Substance use refers to the use of selected substances, including alcohol, tobacco products, illicit drugs, inhalants, and other substances that can be consumed, inhaled, injected, or otherwise absorbed into the body with possible dependence and other detrimental effects. Reporting period varies by grant but typically includes the six-month period ending June 30 or December 31.
