Unique means unduplicated. Individuals should only be counted once. Unique individuals should not be counted in more than one reporting period. Individuals refer to parents or caregivers. Parent is defined as a child’s or adolescent's biological parent, adoptive parent, foster parent, or stepparent through marriage. Caregiver means an individual, including a relative, neighbor, or family friend, who regularly or frequently provides care, with or without compensation, for a child or adolescent for whom the individual is not the parent. This individual may also be designated as a temporary adult caregiver. Discharged means the program participant is no longer participating in services or receiving services directly or indirectly through your program. Program participant refers to an individual who started a program. Unsuccessful discharge refers to individuals discharged for a reason other than successful, such as a discharge due to loss of contact, removal, or a new charge. • Loss of contact means a youth in the program stops communicating with the program or fails to attend required appointments. • Removed means the exiting program participant is no longer participating in the program for failing to follow program rules or treatment recommendations, such as consistently missing appointments, abusive behaviors, or lack of engagement. • Charge refers to a new criminal charge from a law enforcement agency or prosecutor formally accusing an individual of violating a criminal statute, initiating a legal process that could result in conviction, fines, or incarceration. Conviction refers to the court process that determines if an adult committed the act for which they are charged. The term "convicted" indicates that the court concluded the individual committed the act. Beginning of the award refers to the Project Period Start Date for your award. You can find this in your award documentation or in JustGrants. Project period end date is the end of the grant activity period. The project period end date can be found in your award documents or JustGrants. Reporting period refers to the specific timeframe covered by this report.
