RequiredoutcomeFY2026Q13

Tier 1: Collect today

A count of services or people, held in the organization’s own records.

Unique means unduplicated. Families should only be counted once. Unique families should not be counted in more than one reporting period. Families refers to any combination or grouping of parent and child (including pregnant mothers) who are participating in the program. Discharged means the family is no longer participating in services or receiving services directly from or indirectly through your program. Reporting period refers to the specific timeframe covered by this report. Placement stability refers to minimizing the number of moves a child makes from their primary home or foster home during program participation. Stability would be no more than one change in placement during program participation.
